For a cash-pay or private-pay specialty practice, the clearest differences are in the published terms. PatientGain's pricing details page lists subscription plans from $899 a month with a 12-month commitment, plus an 18% management fee on ads. Our Proof Pilot is 30 days for a flat $2,500, with ad spend untouched by us during the pilot and the accounts in the practice's name from day one.
We are Fill The Lobby, so read this with that in mind. Everything here about PatientGain comes from its own pages, linked and checked on 16 October 2026. Most prices are starting prices; confirm them in writing.

What PatientGain says it offers
Its home page describes a marketing automation platform: a website, a lead-funnel CRM, and communication and workflow apps, covered by a BAA. For its PLATINUM service, its pricing details page says its staff and AI-assisted systems handle most of the work, including the website, SEO content, newsletters, social media and ad campaigns.
Price and commitment
PatientGain's all-pricing page has two grids. The one labeled "12-Month Contract" covers GOLD, PLATINUM and PLATINUM PLUS, which its pricing details page lists from $899, $1,699 and $2,499 a month. A few of its other pages show lower figures for the same plans, so ask which price applies to you. That page says subscription services have "No upfront cost, but there is a 12 month commitment." At those starting prices, 12 months comes to $10,788 for GOLD and $20,388 for PLATINUM, before ad spend. The other grid, headed "Full Website Ownership With Setup Fees," covers Foundation from $800 a month, Growth from $1,600 and Competitive from $4,000, each plus setup fees.
Shorter terms come with a setup fee. Its apps pricing page says month-to-month or 3 to 6 month contracts require one, and the pricing details page says "Month to month contracts have higher fees." Its custom services run month-to-month, typically $10,000 to start and $2,500 a month for one location. We found no published amount for the other setup fees, no published terms for leaving early, and no trial of the full service (checked 16 October 2026). Ask for each in writing.
Our pilot is 30 days for one flat fee of $2,500: "One fee, no retainer behind it." Thirty days is the whole commitment, so the most a practice commits before it sees its own booking report is $2,500 plus its ad spend. In week four we report what was booked, where each booking came from and what each cost. Then the practice chooses a long-term plan, priced by clinician count and given in writing first, or walks away and keeps everything built.
Ad spend is extra with both. PatientGain's ad management page lists a fee of 18% of the monthly ad budget, so $180 on $1,000, dropping to 16% at $20,000 a month and 12% at $50,000. Our pilot fee stays flat whatever you spend: ad spend goes from your own accounts straight to Google and Meta, and we never touch it or mark it up.
What you keep if you leave
PatientGain's pricing details page sets out ownership by plan:
- Website and content. On a no-setup-fee plan, the client keeps the content it supplied, in its original format, but not the website or content PatientGain created. At the end of the 12-month contract it can buy the website for the standard setup fee, which "is always disclosed upfront in the contract," or buy just the content. A client who walks away without paying "cannot copy, download, or reuse anything from the website created by PatientGain."
- Apps and work-product. Its apps, software and IP stay PatientGain's and "cannot be purchased or moved," and "work-product all belong to PatientGain." After canceling, the apps can be used for a monthly fee.
- Domain and patient data. The client always owns its domain and its patient data, and can export its CRM contacts.
- Ad accounts. Ownership of "any accounts" is stated for CUSTOM PLATINUM clients, whose setup fee starts at $7,000, and full CUSTOM clients. We found no statement of whose name the ad accounts are in on the subscription plans (checked 16 October 2026).
With us, ad accounts, data, pages and history are in the practice's name from day one, and a practice that walks away after the pilot keeps everything we built.
After the click: who answers the phone
Describing its PLATINUM service, PatientGain's pricing details page says "the practice remains responsible for answering calls, responding to patient inquiries and leads that require staff involvement, scheduling patients, and providing care." Its missed-call texting page describes an app that waits a set time and then texts the caller. We found no named service for reactivating lapsed patients (checked 16 October 2026).
We run eight services as one system. Ads bring the patients; the other seven stop them leaking on the way to the calendar: a missed call, a thin Google profile, an inquiry nobody followed up. Calls answered, follow-up, reactivation and reviews are four of the eight. A missed call is an ad click already paid for, so ask who picks up when the ads make the phone ring.
Who each one is built for
Beyond the practices its home page lists, it sells marketing packages for dentists and lists an insurance app for questions such as "Do you take ABC insurance?"
We work with cash-pay and private-pay practices, in eight specialties with a page each: private-pay therapy, psychiatry, ketamine and Spravato, IOP and PHP, concierge and DPC, functional medicine, weight loss and GLP-1 and gastroenterology. Every tactic we sell runs first on Shifa Therapy and Counsel Clinic, the two platforms our founders own and run, with their own money.
Compliance: what each one puts in writing
PatientGain's pricing details page says GOLD, PLATINUM and PLATINUM PLUS clients get its standard BAA, and that ad-tracking data is "obfuscated" and sent server-side.
Our compliance page sets the rules for every client: a BAA signed before any patient data moves; no patient health information in ad platforms; patient lists never uploaded for targeting; ads reviewed against APA, ACA, NASW and state board advertising rules; no patient testimonials and no outcome claims.
Two differences to ask about. PatientGain's PLATINUM page describes PatientReel, an app to collect, edit and publish patient video testimonials; our policy is no patient testimonials. On the pages we checked, we found no statement about reviewing ads against board advertising rules, or about uploading patient lists to ad platforms (checked 16 October 2026). That doesn't mean it has no policy, so ask. We're a marketing company, not a clinician and not your compliance officer.
Questions to ask PatientGain before you sign
- Price. What will my plan cost each month, all in, in writing, including the ad management fee, any setup fee and the website buyout?
- Term. Can I read the terms of service before I sign? Its legal page says new customers can request them. What happens if I leave before 12 months?
- Accounts. Whose name are the ad accounts, analytics and call-tracking numbers in?
- Reporting. Will reports show booked appointments by source, and what each cost?
- Calls and follow-up. Who handles a call we miss, and an inquiry that doesn't book?
- BAA and data. When is the BAA signed? Is any patient list ever uploaded to ad platforms? Are my ads checked against my board's advertising rules, including on testimonials?
Ask us the same questions. Our answers are on the pilot and compliance pages.
When Fill The Lobby isn't the right fit
We're the wrong choice for hospitals and health systems; pharma and device brands; and practices that mostly bill insurance and want volume.
Try the 30-day Proof Pilot: $2,500 flat, with the accounts in your name. Or start with a free audit of how your practice gets patients, or book a 30-minute call.
